First, the surgeon creates a small pouch from the upper stomach — roughly the size of an egg. Then a segment of the small intestine is rerouted and connected directly to that pouch, bypassing most of the original stomach and the first part of the small intestine.
That gives the bypass two mechanisms: restriction (you can only eat small amounts) and malabsorption (some calories pass through unabsorbed). It also rapidly changes the gut hormones that control hunger and insulin sensitivity, which is why most patients with type 2 diabetes see their blood sugar normalize within weeks — often before significant weight loss has occurred.
